It does take practice. Meditation, contemplation, communication, praying all assists. Clear thinking with a clear mind/heart/soul gives both sender and receiver in communication patterns a better form of outcome.
When the energy patterns do not match when trying to communicate, the words spoken generally do not get heard correctly especially the way the sender wants to have the words received. There can be a mental block, a spiritual block, a soul block, any-other kind of block that interferes with proper communications. Psychic interferences also intrude inside the person who is trying to communicate, being a good listener is important. It takes allot of patience to hear what is really being stated in words, versus the deception of what is said or heard.
The practice of reading the body language with the spoken words is a form of discernment. Also paying attention to patterns and cycles of energy between a couple or partners, or any-kind of a relationship.
Most of the time when emotional energies are very intense in terms of feelings about the person during a conversation can trigger receptiveness or not. Therefore, it is wise to be very clear about the timing when communicating.
This is an art and science related to communication skills, related to any relationship, partnership, friendship, coworker, family, etc. These kind of interpersonal relationships are very much different. Therefore, good communication skills and good intentions go along way with clear thinking that brings less confusion. No amount of creating confusion, creating deception will ever clear the confusion, it causes more confusion.
