Welcome, my name is Patricia Bridges, LPN, as founder of
Bridges Eldercare Consulting, my dedication to nursing and
my compassion for the elderly began more than 30 years ago as
a nursing assistant while still in high school. The
experience and knowledge acquired during my nursing career
is immeasurable. However, it is the passion and dedication I have for the
elderly that inspired my vision of geriatric consulting.

What is a Geriatric Care Manager
A geriatric care manager is trained and experienced in the assessment, coordination, and monitoring of services for older adults and their families.
A geriatric care manager allows you the comfort of knowing your older family member or friend is receiving the care that he or she needs while leaving you the time to pursue your own personal, family and career commitments.
Care managers often assist families with long-distance care giving; by working with attorneys, financial planners and trust officers, as well as, physicians, and other professionals to help meet the needs of the clients.
Our goals
First, to help the older person remain independent and at
home as long as possible.
Second, to assist families in the planning and care of older
relatives when independence is no longer possible.
Finally, they do whatever is necessary to locate the services
that meet the older person's specific needs and explain options
available to the client and all concerned.
